Immunizations

Central ISD requires at least one TB skin test after the age of 3 before student can be registered, in addition to all state mandated immunizations.

Juniors/Seniors:
For your immunizations to stay in the national ImmTrac database, the below form needs to be filled out and returned to the Health Office or the Angelina Cities and County Health Department.
