MEDICATION AT SCHOOL
 
School personnel may administer prescription medications ONLY. All prescription medications must be properly labeled by a pharmacist and in the original container with a current prescription date. 
 
A physician's form MUST be filled out and signed by both the prescribing physician and parent for any medication to be given longer than two weeks. 
 
The health office does require Emergency Action Plans (EAP) for certain medical problems such as Asthma, Life-Threatening Allergies, Seizure Disorders, Etc. Please see the "Forms" tab for required EAP forms.
